Chronic Disease Management
An important part of primary care is working with your provider to manage chronic illnesses, like diabetes or hypertension. At the Libby Clinic, we have a lot of experience managing numerous chronic illnesses both independently and in conjunction with your specialists when needed. One of the duties of primary care is to serve as your “medical home” to help coordinate between different specialists and make sure that you stay on-track with your health goals. It is important that even if you have a specialist managing your care (like a cardiologist or endocrinologist), that you still maintain a relationship with your primary care provider, as they are the ones who look at the whole picture of you and make sure things stay on-track.
We will also help you with managing your medications, particularly when they are prescribed by different specialists. The average American adult who takes medications takes an average of 4, and as we get older that list usually gets longer. It is important that these be evaluated regularly by your primary care doctor to ensure there are not interactions or adverse effects.
